These are Aztec Concrete Accessories Castle Chair rebar supports.
The rebar can be set at 1 1/2" or 1 3/4" high.
I use these for radiant heated slabs because they work well to support the rebar when you have rigid foam underneath the slab. In my opinion these are the best for that application. I purchased in bulk and am nearing retirement.
I used 60 for a 12 x 14 slab on the job shown (1/2" bar, 12" x 12" grid) and wheeled concrete on plywood placed over the grid with no issues. Could use less for a direct pour. How many you use is up to you it's your job. Some of them have minor surface dirt see last photo.
